What is e-GFR?
An individual's estimated glomerular filtration rate (eGFR) can be calculated based on an individual's serum creatinine level, age, weight and race. There are currently 19 million Americans in the early stage of chronic kidney disease (CKD). Because serum creatinine values in the upper reference interval to moderately elevated range are difficult to relate to kidney function, eGFR provides a readily available laboratory result for identifying individual's with chronic kidney disease.
The National Kidney Disease Education Program (NKDEP) is urging laboratories to "automatically report eGFR when serum creatinine is reported” as a means of identifying those people with chronic kidney disease (CKD) who might otherwise go undetected and untreated. NKDEP recommends laboratories use one of two MDRD Study equations—the original traditional calibration MDRD equation or the recently developed IDMS-traceable MDRD equation--depending on whether or not serum creatinine methods have been calibrated to be treaceable to an isotope dilution mass spectrometry (IDMS) reference method.
State Legislative Efforts Mandating the Reporting of e-GFR
Over the past two years, model legislation has been introduced in a number of states that would require clinical laboratories to calculate and report an individual's eGFR when a serum creatinine test is ordered—even if the physician does not request it. To date, four states have enacted such legislation. AACC is opposed to this legislation.
AACC AND e-GFR
Although AACC opposes the state legislation, it does not oppose the NKDEP initiative to promote the use of the eGFR. AACC encourages its members to learn about the calculation, read the scientific literature, consult with their physicians and consider implementing it within their own institutions.
