Is There a Useful Test for Laboratory Assessment of Cortisol Replacement in Adrenal Insufficiency?

This is a very important and interesting topic.I was once consulted on UFC in hydrocortisone therapy but the physician wanted UFC for dosage adequacy for Hydrocortisone replacement in AI.In normal people UFC and serum Cortisol levels correlate pretty well.But in patients with Hydrocortisone therapy this correlation probably does not hold true.If UFC is used to adjust dosage of hydrocortisone,the interpretation might be erroneous as UFC is falsely high.But if the question is of using UFC in interpreting absorption of hydrocortisone then may be serum cortisols levels could be a better choice than UFC.My experience is that we clinically evaluate the hydrocortisone dose requirement by periodic measurement of blood pressure,evaluation of signs and symptoms of AI.
